1.   Applicant must be of immediate Portuguese ancestry such as mother, father, maternal or
      paternal   grandparent.

2.   Parents must be members of the Portuguese American Club for a minimum of three (3) years.

3.   Applicant must be awaiting acceptance or already have been accepted as a full time
      student to any two or four year college or university; or post high school career
      oriented education program requiring tuition and leading to certification.

4.   Applicant must be a graduating high school senior, class of 2008.

5.  Applicant must live within the immediate area of the Lehigh Valley.  Two (2)     $1,000.00
     scholarships will be awarded.

7.  Scholarships will be paid directly to the recipient upon successful completion of one semester
     of college.  The student shall submit a copy of the first semester cumulative grade report in conjunction      with a copy of an official receipt signifying that the tuition has been paid on a full-time basis for the      second semester at the college or university or career-oriented education program.  The recipient must      comply with both requests prior to disbursement. Please retain this instruction sheet in order to comply      with rule 7 and use the above address for all correspondence.

8.  Only completed and signed applications will be considered.

9.  Any misleading information will automatically disqualify the applicant.

10. All applicants will be notified by the committee, whose judgment is final.

11. Return the application sheet, transcript and pictures to Portuguese American Club at the
      above address On or BEFORE MAY 20, 2008.
